'Soft Teeth' is a Myth: The True Origins of Dental Caries

"I brush every day, but I get cavities because I inherited my mom's soft teeth."

It is arguably the most common refrain heard in dental clinics worldwide. The belief that one is genetically doomed to suffer from rampant tooth decay provides psychological comfort, absolving the patient of responsibility.

However, from an anatomical and biological standpoint, the concept of "soft teeth" is almost entirely a myth.

The Hardest Substance in the Human Body

Tooth enamel is not soft. It is an intricate, highly mineralized lattice of hydroxyapatite crystals that is significantly harder and more durable than human bone.

While there is an extremely rare genetic condition called Amelogenesis Imperfecta (which affects roughly 1 in 14,000 people and causes enamel to form improperly), 99.9% of the population is born with structurally sound, hyper-dense enamel.

If Not Genetics, Then What?

If "soft teeth" is a myth, why do some people seem exponentially more prone to cavities despite brushing? The answer lies in the environmental ecosystem of the mouth. Caries (tooth decay) is not a genetic defect; it is a transmissible, diet-driven bacterial infection.

  1. The Oral Microbiome: You inherit bacteria, not "soft teeth," from your parents. If your mother had high levels of Streptococcus mutans (the bacteria that causes decay), she likely passed that exact bacterial strain to you by sharing spoons or kissing you as an infant.
  2. Dietary Frequency (The Stephan Curve): It is not how much sugar you eat, but how often. If you sip on a single sugary coffee over the course of four hours, your oral pH is pinned in a highly acidic state for half the day, dissolving the hard enamel at terrifying speeds.
  3. Salivary Flow: Medications, stress, or mouth-breathing can reduce your saliva output. Saliva is the mouth's only defense mechanism against acid. Without it, even the hardest enamel will swiftly disintegrate.

Your genetics may influence the shape of your teeth or the volume of your saliva, but they do NOT predestine you to decay. Cavities are 100% preventable by manipulating your environment.
